The American Dental Association recommends a preventative dental appointment every 6 months. We are passionate about providing quality systemic care to every patient. We now are offering this plan for those who are not covered under insurance and would like to continue their dental health.

Adult

Ages 18 & Older
$544/annually
A 20% savings over regular fee of $680

Plan includes:

2 Periodic exams
2 Prophylaxis (cleanings)
1 Set of routine preventative x-rays
2 Fluoride treatments
1 Periodontal screening
2 Oral Cancer Screenings

Child

Ages 17 & Under
$384/annually
A 20% savings over regular fee of $480

Plan includes:

2 Periodic exams
2 Prophylaxis (cleanings)
1 Set of routine preventative x-rays
2 Fluoride treatments
2 Oral Cancer Screenings

Periodontal Patients

$952/annually
A 20% savings over regular fee of $1,190

Plan includes:

2 Periodic exams
4 Periodontal maintenance therapy visits
1 Set of routine preventative x-rays
2 Fluoride treatments
1 Periodontal screening
2 Oral Cancer Screenings

Receive a 10% Discount

In addition to regular dental visits, members of the Dental Discount Plan will receive a 10% discount on the following treatments:

Sealants
Crowns
Implant Restoration
Panorex (every 5 years)
Dentures

Arestin
Bridges
Night Guards
Full Mouth X-Rays (every 5 years)

Velscope
Fillings

Root Planing and Scaling

Terms and Conditions

This is NOT a dental insurance plan. This is offered to patients without dental insurance and is a savings option.
The plan is annually renewable every 12 months and the fees are non-refundable.
To receive the 10% discount, all previous balances MUST be paid in full at the time services are rendered.
Care Credit does not apply.
In the event of a broken appointment with less than 24 hours’ notice; we will reschedule your appointment with a warning. The second missed appointment, we will assess a broken appointment fee of $73. A third broken appointment will result in dismissal from the practice as well as all fees forfeited.
